Mind on Health recognises how important psychological support is for people who live with disabilities. The practitioners at Mind on Health support NDIS by providing services to eligible participants to increase their daily lifestyle and quality of life.

Located in the Gold Coast, our experienced practitioners take an individualised approach to actively supporting our participants and understand that each individual have different and unique needs and goals through-out their journey.

We deliver services under ‘Capacity Building Support’ with the first category being ‘Improved Daily Living Therapeutic Support’. This aids in assessment, training or therapy to help increase your skills, independence and community participation. These services can be delivered in groups or individually. The second category our practitioners offer support with under the Capacity Building is ‘Improved Relationships’. This will aid in developing positive behaviours and positive interactions with others.

Capacity Building Supports help build your independence and skills to aid in the pursuit towards your short and long-term goals.

Mind on Health also offers Behavioural support for participants either in clinic or mobile. Our practitioners use an evidence-based approach and are fully qualified and equipped with the correct tools and knowledge when providing supports for participants with challenging behaviour. Our practitioners will not only support the participant, but also their support team.

There are many benefits to seeing one of our experienced practitioners to support you in your Capacity Building Supports. Our practitioners will always provide a safe space for participants to discuss any problems without judgment whilst maintaining a high level of confidentiality at all times.

The practitioners at Mind on Health will always show empathy, kindness and understanding to each individual need.

It is important to advise that Mind on Health also offer Cognitive Assessments that can often be covered by NDIS, or used in the application process of NDIS. These assessments come under the Capacity Building Support and are as follows:

  • Wechsler Intelligence Scales for Children, Fifth Edition (WISC-V Australian and New Zealand Adaptation)
  • Wechsler Adult Intelligence Scales, Fourth Edition (WAIS-IV).
  • Adaptive Behaviour Assessment System (ABAS-II) (to be completed by parents/carers).
  • Wechsler Individual Achievement Test – 3rd Edition (WIAT-III).
  • Test of Nonverbal Intelligence, Fourth Edition (TONI-4)
  • Austism Spectrum Rating Scale (ASRS)
  • Austism Diagnostic Observation Schedule, Second Edition (ADOS-2).

ELIGIBILITY:
To access any of the support NDIA offer, you first need to be approved. The eligibility and application process can be a time-consuming journey. Mind on Health is here to offer the correct guidance and tools to get you on your way.

If you have already determined you are eligible there are plenty of options to get the ball rolling.

  • Calling NDIS on 1800 800 110. This is a verbal application process so please make sure you have ALL relevant documentation with you.
  • Complete an Access Request form and send it to the NDIS.

NDIS advises that if you need to apply, please contact your local area coordinator, early childhood partner or the NDIA office.
When applying you must also send NDIS evidence to prove:

  • Your age
  • You are an Australian Citizen or permanent resident
  • You live in Australia permanently
  • Your disability

Eligibility is approved within 21 days of receiving ALL relevant information and documents to support your application.
